Welcome! I hope you find this Photoshop tutorial agreeable.
1.) First off we need to select a picture to work with. If I want a good quality picture of a model I suggest using Google or your favorite search engine to find a stock photo website. Here is the photograph we will use for this tutorial:

2.) The next step is to get rid of that mouth. In the main toolbar to the left you will see a icon that looks like a band-aide, which is called the Healing Brush Tool, or simply hit J on your keyboard to hotkey to it. When the tool is selected hold down the alt key and click on a part of the face near the mouth, and make short strokes over the mouth. This will cause the mouth to now look like the skin that you selected. Keep doing this unless were the mouth use to be now looks just like the rest of the face. When doing this make sure you alt+select parts of the skin nearest to the part of the mouth you are manipulating to keep the tone of that section of the face. Here is what I have at this point:

3.) Now we get to give the individual a black eye. Hit O on your keyboard to select the menu where the burn tool resides. Then at the top were the burn tools options are make sure the range is set to midtones, and the exposure is set to 50%. I would also suggest you use a fuzzy looking brush around 50 or so. I chose “Soft Round 45 pixels.” Then brush around her eyes to make the effect as if she was just punched in both eyes. Pimp isn’t it? Keep doing that until you get the desired effect, this is what I ended up with:

4.) Grab the texture I used and open it up in Photoshop. When the texture is opened ctrl+a to select the image, then ctrl+c to copy the image. Now on your picture of the woman hit ctrl+v to paste the texture as a new layer over the lady. In the layers panel click on the texture layer. Then change the opacity of that layer to around 65% or 70%. Now we need to change the layer mode to vivid light in order to make it go over the actual face and not the whole image. It still looks weird though because the color of the texture is too much like the rest of the face, so what we need to do is use the hue and saturation feature by going to Image+Adjustments+Hue and Saturation or hit ctrl+u. Now we want to give her a greenish white look to make her look dead and decaying. Make sure that the texture layer is the one that is selected, and change your hue to +160 and leave the other 2 options at 0 then click ok. This is what your image should look like now:

5.) Now to stitch up those lips.. To do this we need a stock photo of something that has been stitched. This can be done with a picture of 2 pieces of carpet that have been glued together, but that doesn’t look to good to me.
To have a more realistic feel I use a stock photo of some real stitches which can be found here. Now, open it up and hit ctrl+a to select it, then ctrl+c to copy & paste it on the photo of the girl with ctrl+v. The stitches need to be over the texture layer and the face layer. In the layers panel drag the stitches layer over the textures layer so that the texture is below the stitches. You have probably noticed already that the image of stitches is huge. To resize just the stitches layer you can go to Edit+Transform+Scale then drag the now selected corners of the stitches layer to make it smaller. I made it the size of what the old lips use to be to make it more realistic. However, you can make it slightly larger to give a Nightmare before Christmas effect if you wish.
Use the layers panel to change the layer mode of the stitches to Luminosity. However, the lips don’t fit in with the face at all, so you need to select the eraser tool from the toolbar. To do this either click on the icon that looks like an eraser or just hit E. At the top left you will see a option that says brush which lets you choose the brush size. I would choose the smallest brush size possible, and make sure you pick a fuzzy looking one. Now erase all of the extra, which is everything but the stitches themselves. When your finished your picture should look similar to mine shown here:
